in uncomplicated mitral stenosis the 2nd heart sound is not loud
the 2nd heart sound becomes loud only if mitral stenosis is complicated by the devlopment of pulmonary hypertension

In a normal heart the pliant mitral valve floats up during diastole and is partially closed when systole begins
in mitral stenosis the thickened mitral valve does not float up and at the beginning of systole is almost fully opened
thus in mitral stenosis the valve moves a greater distance when shutting and therefore the first sound is louder
if the patient is in sinus rhythm , atrial contraction , which is stronger due to left atrial hypertrophy , pushes the valve futher away and further increases the intensity of the first heart sound
