anaemia means the haemoglobin level is low
megaloblastic means there is megaloblastic erythropoiesis ( erythropoiesis where nuclear maturation is slow compared to cytoplasmic maturation )
in megaloblastic anaemia, lack of b12 or folate slows nuclear maturation
fewer red cells are produced
hence the haemoglobin level is low
however each cell contains more haemoglobin than normal hence MCV , MCH are high
in addition to reduced production the macrocytes have an increased rate of destruction as their abnormal size and shape interferes with passage through the microcirculation
Notes in KEYS to SUCCESS in Medicine have been designed for UNDESTANDING and REMEMBERING We have concentrated on using pathology , pathogenesis and pathophysiology as the basis of understanding We have used a strucutre similar to the one used in ACES for PACES to facilitate remembering We will welcome your feedback on all aspects of our endeavour ( websites , book, blogs, courses ) as we are making a big effort to provide good quality medical education regards Ajith Jayasekera
