anaemia means the haemoglobin level is low
megaloblastic means there is megaloblastic erythropoiesis ( erythropoiesis where nuclear maturation is slow compared to cytoplasmic maturation )
in megaloblastic anaemia, lack of b12 or folate slows nuclear maturation
fewer red cells are produced
hence the haemoglobin level is low
however each cell contains more haemoglobin than normal hence MCV , MCH are high
in addition to reduced production the macrocytes have an increased rate of destruction as their abnormal size and shape interferes with passage through the microcirculation
